Day 4 — Alcohol Cravings: What They Are and How Long They Last

Yesterday ended with a warning label: the groove in the grass doesn't vanish when you stop walking it — it waits, and some evenings it calls. Today is about the call. What a craving actually is, which of three doors it walks through, why it feels like a command — and why it's really a wave.

And then it arrives — not a thought so much as a pull, somewhere between your chest and the back of your mouth. One drink would land right now. It feels less like an idea and more like information. Like your body is reporting a fact.

It isn't a fact. It's a forecast. And by the end of this page you'll be able to watch it the way you'd watch weather.

The mechanism

A forecast, not a need

On Day 1 you met the highlighter — dopamine marking what matters. Here's the strange thing decades of research turned up: the system that makes you want something and the system that makes you enjoy it run on different wires. With repeated drinking, the wanting wire gets stronger while the liking wire quietly fades. That's why people describe craving something they no longer even enjoy. Not a contradiction. Two systems, drifting apart.

So what is a craving, physically? It's the highlighter firing at a cue. On Day 3 you learned that dopamine moves from the drink to the things that predict the drink. That firing is a forecast: relief is available, go get it. A craving is what that forecast feels like from the inside — your brain predicting, with total confidence, how a drink will feel, based on old data.

Hold onto that word. A craving is a forecast, not a need. Forecasts can be wrong, and this one is running on data from a brain that has since changed its mind.

The science behind this

Kent Berridge and Terry Robinson (University of Michigan) spent thirty years pulling "wanting" and "liking" apart in the lab — their incentive-sensitization theory is why we can say the two run on separate circuitry, and why wanting can grow while liking shrinks.

Wolfram Schultz showed that dopamine neurons fire to predictions of reward, not just reward itself — the forecast machinery this page is built on.

Where it comes from

Three doors it walks through

Cravings arrive through one of three doors, and it helps to know which one you're standing in.

The cue door. The world reminds you. A clock, a street, a smell, a glass, a certain friend, the sound of a cork. Your emotional and context memory recognize the pattern in a fraction of a second — well before you've consciously noticed anything — and hand the highlighter its cue.

The stress door. A feeling reminds you. Tense, angry, sad, lonely, bored, ashamed. In a brain that has used alcohol to switch the stress alarm off, the alarm now has a learned off-switch — and when it rings, the brain reaches for it. This one doesn't feel like I want the buzz. It feels like I need this to stop. Researchers call it relief craving, and it's the door most people relapse through.

The body door. Your body reminds you. Hunger, tiredness, pain, dehydration, the leftover chemical wobble from Day 2. A brain that spent years answering discomfort with a drink often labels any discomfort "want a drink." Recovery culture's checklist for this door — HALT: hungry, angry, lonely, tired — turns out to be neuroscience.

The science behind this

Antoine Bechara and Nasir Naqvi found that smokers who suffered stroke damage to the insula — the fold of cortex that reads body states — often lost the urge to smoke almost overnight, saying their body "forgot" it wanted cigarettes. Craving lives partly in the body's readout, not just in your thoughts.

Lorenzo Leggio's group showed that ghrelin, the hunger hormone, directly increases alcohol craving — an empty stomach genuinely makes the wave louder. Rajita Sinha at Yale mapped the stress door: how tension and low mood reliably switch craving on.

Why it grips

The picture is the pull

A thought becomes a craving in two steps, and the second step is where the intensity comes from.

Step one is an intrusion: the thought just shows up, delivered by whichever door. Everyone gets these. Step two is elaboration — your mind starts rendering it. The cold glass. The first sip. The exhale. That vivid sensory picture runs in working memory, and the picture is the pull. The more detailed the rendering, the stronger the craving.

Which hands you a lever: working memory is small. Fill the screen with something else visual — a puzzle, a drawing, a detailed imagined scene — and the rendering loses its bandwidth. Change the picture and the pull drops.

The science behind this

David Kavanagh, Jackie Andrade and Jon May built the Elaborated Intrusion Theory of desire — the intrusion-then-rendering model this section describes. In a neat test of it, Skorka-Brown and colleagues (2015) found that a few minutes of Tetris measurably cut craving strength: the falling blocks occupy exactly the visual workspace the rendering needs.

Matthew Lieberman's affect-labeling work explains a second lever you'll use tonight: putting a plain name on a feeling — craving, wave, stress door — measurably dials the emotional circuitry down and hands the moment to the prefrontal brake.

The shape of it

A wave, not a ramp

Here's the part almost nobody is told. A craving that isn't fed doesn't climb forever. It rises, peaks — usually within minutes — and fades on its own, most often inside ten to thirty minutes. From the inside it feels like a ramp: this will keep building until I drink. That's the forecast talking. Measured in the lab, it's a wave.

Only one thing turns the wave into a ramp: a drink. Drinking at the peak teaches the brain that the forecast was right, and the next wave arrives taller and more confident. Riding it out teaches the opposite — and that lesson is physical. Every wave you surf is a rep for the brake and a correction to the forecast.

Two honest warnings, so you don't mistake them for failure. Cravings can get louder around weeks four to eight before they fall — researchers call it incubation, and it's normal. And random flickers can show up months later, out of nowhere. A flicker is not a relapse. It's an old groove clearing its throat.

Expect the loudest waves between days three and ten, and don't be alarmed by an incubation bump around weeks four to eight. Louder is not stronger — it's the forecast getting desperate before it goes quiet.

If you're cutting back

One rule protects everything: never drink during a wave. A drink taken at the peak is a "the forecast was right" rep — it deepens the groove precisely when you obey the call.

Decide your drinking occasions in advance, in the morning, on paper. A planned drink and an answered craving are chemically different lessons.

A note on safety

First, the warning that leads every page of this series: if you've been drinking heavily every day, stopping suddenly can be physically dangerous. Shaking, sweating, a racing heart, confusion or seizures after stopping are medical symptoms — get medical help rather than pushing through.

Today's addition: a "craving" that comes with those body symptoms isn't the wave this page describes — it's withdrawal wearing the wave's coat, and it belongs to a doctor, not to willpower. And if cravings stay heavy for weeks, ask a doctor about medications like naltrexone or acamprosate. Using the brace is not cheating.
